Guardian Collar Tag & Guardian VIP Ordering

This form may be used to register your initial pet in either the Guardian or Guardian VIP programs. If you wish to order by phone or register an additional pet, please call 800-APA-PETS (800-272-7387). You will receive confirmation by email and your full membership package in 5 - 7 days.

All information is kept strictly confidential! Your information is never sold, loaned or given to any other organization or business.  Our database even restricts personal information from all employees except supervisors (our staff is very trustworthy, this is merely a high security policy).  Please remember, complete information is important to your pet's safety.
